An In-Depth Look at 'Community Guide' and 'Agency With Choice'-SESSION 1

I/DD, COMMUNITY GUIDE, & AGENCY WITH CHOICE PROVIDERS

The purpose of this training is to introduce the various components of the Community Guide service, and to demonstrate, explain, and discuss the roles of the Community Guide provider and the ECBH Care Coordinator. The training will include discussions of how Community Guide supports Waiver Participants in developing social networks and connections within local communities, as well as provide transitional services to individuals seeking independent living. The targeted audience for this training includes ECBH Care Coordinators, Managers, Community Guide providers (Management and Community Guide staff), as well as Agency With Choice providers
