System of Care is a nationally recognized, best practice approach to delivering services and supports. System of Care is family driven, youth guided, and culturally competent. System of Care builds on individual and community strengths.
System of Care has three main parts:
-
Community Collaborative which fosters collaboration among families, agencies, private mental health/substance abuse providers, civic organizations, the faith community, and other community resources. This group works to ensure that the needed services and supports are available in the local community, and it supports the Child and Family Teams.
-
Child and Family Teams who are the heart of the System of Care and who develop around each child and family.
-
Person-Centered Plans which are developed by the Child and Family Teams. They are strength-based and culturally competent.
